MEDICAL RECORDS DEPARTMENT

INTRODUCTION

Medical records are essential component in the effective management of patients’ health care. The Medical Records contain information needed to plan, provide and evaluate the care given to the individual. It serves as a tool for communicating information to all health personnel who deal with the patient and contributes to the continuity of patients care. To fulfill these functions successfully, requires not only that the medical record meet certain minimum standards but also a system for making the information contained in the records available when needed.

PURPOSES OF MEDICAL RECORDS

1. Serves as a basis for planning individual patient care
2. Serve as a basis for analysis, study and evaluation of the quality of care rendered to the patient.
3. Provide data for clinical or epidemiological research and for education
4. Provide data to assist in protecting the legal interest of the patient, the institution and the staff.
5. Serve as a source of information for management and planning .
