DEPARTMENT OF OPTHALMOLOGY

The Department of Ophthalmology at KCMC was one of the original departments when the centre was opened in 1971. A separate wing of the hospital houses the ophthalmic ward and theatre suite, as well as eye outpatients, optical facilities, and administrative offices. This was built with donations from Christoffel Blindenmission (CBM) in collaboration with the Good Samaritan Foundation of Tanzania (GSF).

Job Title: Diabetic Programme Manager
Job Location: Kilimanjaro Christian Medical Centre, Moshi, Tanzania	Department: Eye Department

Background of the programme: Diabetic Programme is a six years initiative implemented by KCMC Eye Department in collaboration with Christofel-Blinden Mission, 14 selected health centres within Kilimanjaro. Diabetes is a growing problem in the developing world and the number of people with diabetes in Eastern Africa is expected to reach 1.4 million by 2025. Many of them will develop complications such as blindness, suffer strokes from poorly controlled blood pressure and undergo amputations which can be prevented through the appropriate screening and treatment. The Diabetic Programme has so far made significant achievement towards improved access to health services for people with diabetes through trainings of health care providers, creating linkages and coordination between the different components of diabetic care and rehabilitative services and dissemination of information in the community at large. The programme has been effective in highlighting how effective management of diabetes can reduce the prevalence of blindness and other complications and has established a Diabetic Tracer bank where patient information is stored in a central data base..

Position description:
The programme manager is to lead the way forward in reinforcing and reaffirming the established positive model of preventive and curative care for diabetes in Kilimanjaro Region through coordinating activities for the comprehensive and sustainable diabetic program. This will involve training workshops, the screening program, community education and the establishment of diabetic clubs through the participatory action of stakeholders using organisational structures already in place in the community and at the referral hospital.

Key Responsibilities:

Assist in doing needs assessment of diabetic patients and diabetic service providers.
Creating information sheets that increase referral and accessibility of services to high risk patients.
Develop PR material for public awareness campaigns, and plan for radio and poster distribution throughout the year.
Strengthening capacity of local partners- identification, and skills improvement, planning meetings, and annual reviews.
Setting up and improving counselling services for diabetics. Training and re-training community workers and health care professionals at the hospital and in the community in counselling procedures
Provide training workshops for health care-workers in the Kilimanjaro Region- on eye complications, other forms of disability of diabetes( feet, kidney and other micro-vascular complications), nutritional lifestyle/education, and rehabilitative services for health care workers, primary & community based workers, doctors, nurses and support staff in the Kilimanjaro Region.
Review and refresher training workshops after 2 years for health care workers at KCMC Hospital, DRS in the 6 districts of the Kilimanjaro Region, and private practitioners
Provide supportive supervision for nutritional trainer and community and hospital based counselors
Report to donors as per funding agreements
Prepare monthly progress reports to be submitted to the relevant hospital departments and AusAID
Monitor the budget for all programme activities and produce financial reports as required

Qualifications:

Relevant academic qualifications i.e. public health, anthropology, nursing or social science with sound knowledge of public health best practices
Masters degree preferred in relevant discipline
Experience in program management: planning, management and evaluation of programme work
Knowledge in diabetes and eye care
Extensive experience in collaborating with a variety of stakeholders- local ministries, community based organisations, NGO's, government entities and the community
Familiarity with project cycle management formats- activity based budgets, and using these as primary source documents for monitoring and evaluation
Experience in using log frames, developing annual plans, preparing narratives and financial reports suitable for co-funding
Valid driving license desirable

KCMC EYE DEPARTMENT ANNUAL REPORT FOR THE YEAR 2009.

Staffing:
In collaboration with Christoffel-Blinden Mission Mr. Richard Hess joined the department in April with the aim of doing on-the-job training to local person who will be doing administrative duties and release head of department from spending much of his time in administering the department.
In mid of August our secretary has been re-categorized to the position of Human Resource Officer and she is now getting on with on-the-job training to take over in January 2011.
The department also manages to get two ophthalmologists Dr. Heiko Philippin supported by Christoffel-Blinden Mission and Dr. Lee Woodward (Paediatric Ophthalmologist) who is working on voluntary basis and gets accommodation support from Eye Care Foundation.

Activities/Service Performed
The concern of Eye Department as one of the departments in KCMC hospital is to influence the East, Central and West Africans by providing medical services such as treating, training and research in collaboration with MoHSW, Tumaini University and KCCO.

The department has provided service to 23,611 patients in whom 16,013 patients were seen in the Eye Outpatient Department and 7,598 patients were seen on outreach.
Cataract as the major cause of blindness has make the surgeons did total number of 1,732 cataract surgeries in which 1,262 were done at base hospital and 470 were done on outreach.
The department also performed 466 trabeculectomy surgeries and provided 344 Laser treatments for Diabetic Retinopathy. Also the department performed 1,277 surgeries including retinal, DCR, evisceration, enucleation, lid rotation which make a total of 3,353 surgeries performed during the year.

M. MED Ophthalmology Programme:

4 Med residents graduated as an Ophthalmologist in November 2009 and 4 M. Med Ophthalmology residents enrolled in the University in October 2009 and make a total number of M. Med resident for the year to be 15.

Vitreo-Retinal Training

1 Ophthalmologist from Zambia completed his Vitreo-Retinal Fellowship at Eye Department.

AMO-Ophthalmology Programme:

In 2009 6 students graduated: 4 students completed the 2 years course of Assistant Medical Officer in Ophthalmology and Cataract Surgery, 2 students completed 1 year Assistant Medical Officer in Clinical Ophthalmology.
The students who enrolled AMO-O programme 2008/2010 commenced their field work in 2009 at CCBRT and Haydom Hospitals and they are expected to return early in 2010.

In October 2009, 10 students joined the school; seven enrolled in the 2years course (Assistant Medical Officer in Ophthalmology and Cataract Surgery) while 3 enrolled the 1 year course (Assistant Medical Officer in Clinical Ophthalmology).

OBJECTIVES/PLANS/GOALS/FUTURE PLANS:

Practical Community based Vision 2020 programme by reaching all villages in the direct service catchments area.
Providing high quality retinal surgery service at base hospital
Proving medical retina service at hospital for diabetic retinopathy patients.
Providing necessary equipment to M. Med upon graduation
Raising public awareness on diabetes-related conditions in all forms of disabilities

ACHIEVEMENTS FOR THE YEAR OF REPORT:

The objective was fully achieved by the intake of 10 students in 2009.

The Objectives of the department include the following:

To Provide high quality eye care to those most in need
To prevent blindness, and treat those with blinding diseases
To teach and train medical and paramedical personnel in the practice of medical, surgical and optical eye care

Outreach Surgery is performed in the Tanzanian Northern Zone in the following centres: Lushoto, Karatu/Mto wa Mbu and Babati Magugu

Direct Referral Sites programme has been established to serve Kilimanjaro and parts of Arusha regions. A team of eye doctors and other eye care providers from KCMC Eye Department in collaboration with Kilimanjaro Centre for Community Ophthalmology (KCCO) visit the community and screen patients. Cataract Patients and others who need surgery are provided with transport to the hospital and then sent back to the community after surgery.

Cataract Surgery is the most effective procedure in relieving blindness due to cataract.
