THE CHILD CENTRED FAMILY CARE CLINIC AT KCMCINTRODUCTION AND BACKGROUND INFORMATION

Kilimanjaro Christian Medical Centre (KCMC) which was established in 1971 is an institution of the Good Samaritan Foundation the owner and founder of the centre. It is located in Moshi, Kilimanjaro region, Northern Tanzania.

KCMC is one of the four referral hospitals in Tanzania. It is a teaching hospital with 16 Allied Health sciences schools and a medical college (Kilimanjaro Christian Medical College of the Tumaini University).The official bed capacity is 450 beds and also it serves 1000 outpatients daily.

KCMC being a referral hospital with 11 clinical departments offers a variety of diagnostic and treatment options for various disease conditions including HIV/AIDS. It has also been training health care staff from northern zone of Tanzania on Care and Treatment of PLWHA.

Due to the fact that HIV/AIDS in our setting is rather a family disease,KCMC thought of  an idea of building a centre for care and treatment of children and their families living with HIV/AIDS under one roof. In September 2005, Elizabeth Glaser Pediatric AIDS Foundation (EGPAF) was awarded a grant under the Rapid Expansion Fund of the US Government to establish a Child Centered Family Care Clinic at KCMC.

The CCFCC was built in collaboration between the Good Samaritan Foundation of Tanzania in collaboration with the Government of America; CDC through Elizabeth Glaser Pediatrics AIDS Foundation (EGPAF). The building was official inaugurated on 6TH December 2007.

WHAT IS CHILD CENTRED FAMILY CARE CLINIC? (CCFCC)
The Child Centered Family Care Clinic provides care and treatment to children infected with HIV/AIDS and their families. The clinic aims to be a model of linking care and treatment of children and their families including primary and specialty medical, social services, Preventing Mother to Child Transmission (PMTCT) and Home Based Care. The services are provided under one roof at the same sitting. The clinic stands as a referral centre for any other complicated cases and is providing training to other health care workers in Tanzania. This clinic is run by a team of consultants, Family Medicine Specialists and nurses (including counselors). Management of CCFCC is by the Director, the Coordinator, the Administrator, Training coordinator and the Nurse Incharge.In addition there is a data entry clerk, social worker and Home based care and Community linkage person, medical record technician and File auditor.
Since CCFCC is fully integrated into the existing hospital structure, this measure will ensure sustainability of the clinic activities.

 VISION
The Vision of CCFCC is to become a model of pediatrics HIV/AIDS care in Tanzania through the integration of care and Treatment, Training and Research.

 MISSION
The CCFCC embedded in KCMC major roles of Health Care, Teaching and Research, envisages creating a future of hope, reducing pain and suffering to families affected by HIV/AIDS through it dignified, high quality, sustainable and affordable child centered care and treatment services.

GOAL
The goal of CCFCC is to become a centre of Excellency in Tanzania and beyond in providing quality, sustainable care and treatment to children and their families affected by HIV/AIDS under one roof at one sitting.

 OBJECTIVES

  1. To provide quality health care and treatment for children and their families affected by HIV/AIDS.
  2. To coordinate/conduct outreach training to health care workers at peripheries hospitals
  3. To link PMTCT to care and treatment
  4. To conduct research in the field of HIV/AIDS.
  5. To provide Home Based Care through community linkage

ACTIVITIES AT CCFCC

A: SERVICE PROVISION

  • Family clinics are conducted on Mondays, Wednesdays and Thursdays
  • Eligibility to the family clinic is two members and above, the entry point is a Child.
  • Exposed babies and their mothers are followed up on Wednesdays.
  • Adolescents clinic are held on one Saturday every month.
  • Pediatrics Care and Treatment clinics are held on Mondays.
  • Social welfare services are provided.
  • Internal and External linkage to CTC and Home based care activities are done.

B: OUTREACH SERVICES

  • Thirteen (13) peripheral hospitals and one (1) Orphanage in Kilimanjaro region are visited once per month.

C: TRAINING

  1. Training through Continuous Pediatrics Education Program (CPEP). Pediatric Consultants and Family Medicine Specialists  from KCMC visit 13 peripheral hospital once per month whereby they conduct bedside teaching, lecture presentations and review of clients in the CTC with doctors and nurses of respective hospitals
  1. Through attachment program
    • It is for those working at CTC i.e. clinicians and nurses.
    • Duration is for two weeks.

OBJECTIVE OF ATTACHMENT

  • To expose the candidate on prompt Pediatric care and Treatment.
  • To familiarize on comprehensive care and treatment for families living with HIV/AIDS under one roof.
  • To update knowledge and skills through mentoring in the clinic

It is expected that after completion of the attachment period, the candidate will be able to run such clinics and disseminate the CCFCC concept to colleagues.

RESEARCH
In collaborate with DUKE UNIVERSITY conduct research on HIV/AIDS.
